Are COVID-19 tests covered? If so, under which benefit? Covered under the wellness benefit – section 14 in your plan’s table of benefits.
Is the COVID-19 vaccine covered? If so, under which benefit? Covered under the wellness benefit – section 14 in your plan’s table of benefits.
Is mandatory/government required hospitalization owing to an asymptomatic case of COVID-19 covered? No – this is a standard exclusion on the plan.
What is the current guidance for how symptomatic/positive cases of COVID-19 are covered – both out-patient and in-patient (hospitalization)? Covered in accordance with your plan’s existing benefits. Please refer to your plan’s table of benefits for details of out-patient and in-patient limits.

Is mandatory/government required hospitalization owing to an asymptomatic case of COVID-19 covered? Yes.
What is the current guidance for how symptomatic/positive cases of COVID-19 are covered – both out-patient and in-patient (hospitalization)? overed in accordance with your plan’s existing benefits, though any in-patient deductible would be waived. Are COVID-19 tests covered? If so, under which benefit? Positive result tests covered under the out-patient benefit. Negative result tests covered under the wellness benefit (Plus and Comprehensive plans only).

Is mandatory/government required hospitalization owing to an asymptomatic case of COVID-19 covered? Yes.

Are COVID-19 tests covered? If so, under which benefit? If recommended by a doctor, then covered under out-patient benefit. Otherwise covered under the wellness benefit.
Is the COVID-19 vaccine covered? If so, under which benefit? Covered under the wellness benefit.
Is mandatory/government required hospitalization owing to an asymptomatic case of COVID-19 covered? Only medically necessary treatment costs will be covered. Normally, hospitalization will not be required for an asymptomatic case. Any hotel fees resulting from mandatory quarantine are not covered.
